Accepting an Award

Students accepting any award(s) administered by the FGSR's Graduate Scholarship Committee need to complete and return the following form to the FGSR within 30 days from the date of the award offer letter.

If this form is not received within 30 days, it is assumed the student is declining the award and the FGSR will offer the award to another candidate:

Note: The Award Winner's Handbook contains many regulations outlining how many awards can be held, required registration status, etc. Students accepting an award must comply with these regulations and are required to print a copy of the Award Winner's Handbook.
